Management of Subluxated Cataracts

چکیده
Surgical management of ectopia lentis is one of the major challenges faced by cataract surgeons today. Ectopia lentis signifies a displacement or malposition of the crystalline lens, irrespective of cause. It may occur congenitally or as part of developmental anomalies, as found in Marfan syndrome, homocystinuria, Ehlers-Danlos syndrome, hyperlysinemia, sulfite oxidase deficiency, simple primary ectopia lentis, and congenital aniridia syndrome.1,2 Pseudoexfoliation syndrome (PXF) is probably the most common cause of adultonset zonular dehiscence. Subluxation may also result from blunt external trauma or iatrogenic zonular dehiscence during complicated cataract surgery.3,4 This article offers five pearls for managing these challenging cases.

PURPOSE To report on the outcome of combined pars plana phacofragmentation, vitrectomy, and Artisan lens implantation in the management of subluxated cataracts. METHODS This prospective, interventional, nonrandomized case series included nine eyes of seven consecutive adult patients with traumatic lens subluxation. Pre- and postoperative data (complete manifest refraction, best spectacle-corr...

PURPOSE To evaluate the safety and efficacy of implantation of a capsular tension ring (CTR) and posterior chamber intraocular lens (PCIOL) after lens extraction in subluxated cataracts. SETTING Department of Ophthalmology,General Hospital of PLA, Beijing, China. DESIGN Prospective case series. METHODS We selected 36 eyes in 34 patients with zonular dialysis <150°. After emulsification an...
متن کاملFibrin Glue Assisted Trans-Scleral Fixation of an Endocapsular Device for Sutureless Trans-Scleral Capsular Bag Fixation in Traumatic Subluxations: The Glued Endocapsular Ring/Segment
Subluxated cataracts secondary to blunt injury are a challenge to treat from the construction of rhexis to IOL insertion. For that reason, we designed a new glued endocapsular ring (ECR)/ segment (ECS) for subluxated cataracts and IOLs for sutureless fibrin glue assisted trans-scleral fixation of the capsular bag. The glued ECR/segment stabilizes the capsular bag intra and post-operatively, all...

Purpose. To compare the feasibility of cataract surgery with implantation of endocapsular supporting devices and intraocular lens (IOL) in subluxated cataract in phacoemulsification and manual small incision cataract surgery (MSICS). Design. Prospective randomized intervention case series consisting of 60 eyes with visually significant subluxated cataract. Method. The patients were randomly dis...
متن کاملGlued capsular hook: technique for fibrin glue-assisted sutureless transscleral fixation of the capsular bag in subluxated cataracts and intraocular lenses.
We describe a technique that uses a capsular hook to obtain sutureless fibrin glue-assisted transscleral fixation of the capsular bag. The hook passes through a sclerotomy created under a scleral flap and engages the capsulorhexis rim, providing scleral fixation intraoperatively and postoperatively. A standard capsular tension ring expands the capsular fornix. The haptic of the hook is tucked i...
